  • A statement, under penalty of perjury, that the information is accurate.
  • Your physical or electronic signature.

Counter-notices

If your content was removed by mistake, you may submit a counter-notice to the designated agent containing your contact info, identification of the removed content, a statement under penalty of perjury that you have a good-faith belief the removal was a mistake, and consent to the jurisdiction of the federal court where you reside.
